I accept Ownership of the above pet and agree to do the following:
I hereby accept possession and responsibility for the pet identified above subject to the conditions of this contract. I hereby release and discharge Goliad Pet Adoption Program from liability for any injury or damage to any person or property caused in the future by this animal and from any causes of action, claims, suits or demands whatsoever that may arise as a result of such injury or damage.
We are pleased that you have chosen to adopt a Goliad Pet Adoption Program friend. Your kindness and patience with your new pet will ensure many years of joy and companionship! Please let us know how you are doing and if any questions should arise, please do not hesitate to contact us.

Spay/Neuter Contract
The Texas Health and Safety (Chapter 829) requires that any animal adopted from a shelter MUST BE SPAYED OR NEUTERED. Failure to comply is a violation of this chapter, punishable as a Class C misdemeanor and up to a $2000.00 fine.
I agree to have this pet spayed or neutered by a licensed veterinarian on or before 6 months from his/her birth. I also agree to provide Goliad Pet Adoption with written confirmation of sterilization signed by the veterinarian who performed the surgery within 7 days of the date required.
